Community participation support is about more than leaving the house. It is about being an active part of your local community in ways that are meaningful to you. Volunteering, joining a local group, attending events, accessing services, or building relationships with people in your neighbourhood.
Aamoon Care provides the support that bridges the practical, physical, social, and psychological gaps so you can participate on your own terms.
This is one on one support to access community based activities. You and your support worker go into the community together. This might include community events, markets, parks, libraries, volunteering, accessing shops and services, sports and fitness, places of worship, visiting friends, or exploring new areas.
The long term goal is to help you build connections and familiarity within your community. That might mean developing a regular routine, like visiting the same cafe every week and getting to know the staff, or joining a club where you build relationships. Over time, these connections can reduce your reliance on paid support for social participation.
Funded under Core Supports, under Assistance with Social and Community Participation. This covers the support worker's time while assisting you. Transport costs may also be covered depending on your plan.
They are closely related. Social and civic activities focus on specific outings and events, while community participation support has a broader scope including building ongoing connections and accessing local services. Many activities overlap.
This particular service is typically one on one. If you want group based activities, we offer group and day centre programs as a separate service.
Depends on the funding in your NDIS plan. Your coordinator will review your plan and help you work out how many hours your budget covers.
